Given the unknown impact of crack preparation practices on the risks for exposure to bloodborne pathogens, crack injection may be an important factor in the current HIV epidemic. While drug users have been injecting crack as early as 1990, crack injection is a hidden practice since few research studies or drug treatment providers ask injectors specifically about injecting crack. The fact that both young and older injectors initiated crack injection throughout the 1990s - increasingly in the late 1990s among this sample - indicates that crack injection remains an emerging practice that may expose new cohorts of injectors to infectious diseases. These findings suggest that HIV service providers, outreach workers, and researchers should ask crack users about mode of administration since smoking is generally assumed. Without more detailed inquiries into the modes of administrating crack, crack injection is likely to remain a largely hidden practice. Consequently, IDUs who inject crack will fail to be identified and targeted for interventions designed to reduce the risk of transmitting bloodborne pathogens and other harms associated with preparation practices particular to crack injection. When a pregnant woman is addicted to drugs, her baby can also become addicted while still inside the womb. As a newborn, the baby then suffers painful withdrawal symptoms, just as an adult going through withdrawal does. Newborns that have been exposed to drugs in utero, or in the mother's womb, experience other problems as well. Those whose mothers smoked marijuana, for example, often have low birth weight, which makes it difficult for them to stay healthy and grow, tremors, and vision problems. Those exposed to narcotics suffer from such disorders as diarrhea, vomiting, sweating, hiccups, rapid breathing, and high-pitched crying. About 30 percent of women infected with the AIDS virus will pass the virus on to the fetus, according to the Centers for Disease Control and Prevention. That was the case for Mary, a former heroin addict, who has been straight for seven years. But when she was on drugs, she contracted HIV from a dirty needle. Her third child, now age two, was born with AIDS. "I watch my baby getting sicker every day," Mary says. "I wake up every day knowing that I didn't just mess up my life, but that I poisoned my baby. I cry every day for her." Most babies with AIDS do not live much past childhood. But most drug-exposed babies do. And as these children grow up, they often have disabilities, sometimes for the rest of their lives. These disabilities can by physical, such as breathing and digestive problems, slow reflexes, and poor coordination; mental, such as being slow learners; emotional, such as rarely smiling or laughing; and social, such as having trouble making friends. Juveniles using drugs or alcohol committed 1 in 10 of the nonfatal violent victimizations against older teens. This was 2-1/2 times higher than the percentage of victimizations against younger teens perceived to be committed by a juvenile who was using drugs or alcohol. Use of heroin in pregnancy also may increase the risk of a variety of birth defects. What is not entirely clear is whether these effects are directly due to the drug itself or related to the poor health behaviors that women who take heroin often have. It may also be caused by the substances that the heroin often is mixed with when it is made. |

Remaining in care at a therapeutic community for an adequate period of time is critical for treatment effectiveness. The time depends on an individual's needs. For most people, the significant improvement is reached at about 3 months in treatment.

Tolerance to a drug takes place when an individual is exposed to the same drug repeatedly and begins to build up an resistance to the drugs effects. The body then adapts and develops a tolerance for the drug. The addiction that is produced is so powerful that it creates cravings in the user. These cravings for the drug are the result of its impact on the individual's memory with feelings of pleasantness and euphoria which the individual has come to associate with the taking of the drug.

An addict is an individual who has a compulsive urge to use drugs, to the point where they feel they have no effective choice but to continue use. An addict will continue their self destructive behaviors in order to feel good or to avoid
feeling bad. It can dominate their mind, and keep them coming back for more. The addiction can be
different for each addict, depending on their vice and the kind of person they
are.
